Billing Requirements - License/Insurance/W-9
Subcontractors must provide the documents required in the attached letter, including their W-9 and Insurance information.
The following form also has a sample Certificate of Insurance.

Subcontractor Pay Application
The attached Subcontractor Pay Application is to be used when submitting billing to PBS. The attached form is in Excel Format.

Permitting - Collier County Water Meter Sizing Form
The attached form is used by the Design Professional for calculation of the Water Meter Size. Collier County uses this form to determine if additional Impact Fees need to be assessed and require submittal for every permit in Collier County (except Ave Maria).
If there is an existing meter, please include the account number for more accurate Impact Fee determination.
The original document including notarized signature of the person preparing the document must be provided for permit to be submitted.
The Water and Wastewater Impact Fee Form also needs to be filled out by the Design Professional and needs to be provided to Public Utilities after the permit has been submitted.

FPL Permanent Power Application
FPL Permanent Power applications are used for areas where power service is provided by FPL (not LCEC). This form is used to establish permanent power to a project without permanent power (establishing a new meter).
